Ball spline breakage on Shaw

Breaking output Shaft on ball spline tail shaft, on Shaw 4 link any Ideas whats causing this ?

Driveshaft or yoke too long, maybe hitting transmission. Yoke not able to go in far enough and breaking parts. Mostly seen this on older transmissions. Transmission output shaft not cut back far enough. Possibly too much angle on u joints. but that would be a WAG.
