What is considered the standard holes on new BSB birdcages on a GRT? Where is a good place to start?

Standard holes are always at 12 and 6 o'clock.

The holes should also be 4.5" from axle centerline.

Should both the center of the bolts for the uppers and the lower bars both be 4.5" from the center of the tube?

yes. Those are the standard holes. The rest are for adjustment.
