I've always done it on the ground .. but .. do you guys set your front end wheels-off like this :

or do you do it on the ground, on the wheels. Seems like I'd get more consistent results up in the air.

If you do it up in the air .. it looks like I just need to drop in some radius rods so I can set the front ride height, put the frame rails approximately at ride height off the ground, and start the setup. Does that sound right ?

Best way I've found was not the ground. Mark the wheel each direction at 20 degrees so you go to the same spot each time. Tires should be turned 20 degrees, not the steering wheel. Unless you can get the car off the ground and have all 4 corners the same as they are when at ride height I wouldn't do it. If the rear is lower or higher it will affect your caster reading although minor.
